1.Once the Forms link is selected, the PAD Form Type questions window displays.
2. Answer the PAD Form Type questions.
3. Review the instructions that display on the screen.

4. Click Go!.
A File Download dialog box displays.

5. Click Open.
The form opens in Adobe Reader.

6. Verify that the information displayed in the Client Information section is accurate.
7. Complete the fields on the form.
8. (Optional) Save the completed form to your local drive.
9. Print the completed PDF form.
10. Sign and date the printed form.
11. Send the completed and signed form together with either the "void" cheque or bank letter to:
